Single choice

Kylie is a 10 year old girl who suddenly developed a bunch of very weird behaviors.  Kylie is afraid of germs and fears that she will become quite ill and die if she comes in contact with any germs.  In order to avoid getting sick, Kylie washes her hands for 10 minutes at a time with a lot of soap and very hot water and she does this over and over again throughout the day. Her hands are raw and red because of this and she has developed sores on her hands so that washing them hurts. But she continues to wash so that she does not get "germs". Once she washes her hands she feels better, but then the desire to wash them again builds up until she has to wash them again so that she can feel better.  Kylie also refuses to touch things, so she carries around a mechanical grabber and grabs everything using that instead of her hands. And she won't eat using utensils and is afraid to touch her own food, so her mother has to feed her by holding the utensils herself and putting food into Kylie's mouth without Kylie having to touch anything.  Kylie's parents have tried to get her to stop and they have tried or suggested many things - wearing gloves, etc - but Kylie won't do any of the things they suggest.  Kylie knows her behavior is excessive and she is perfectly normal in all other areas of her life, but she is very stuck on this idea that her hands have to be clean at all times or she will "get germs and die". Kylie's mother notes that these problems appeared shortly after Kylie got very severe food poisoning but that her fears are not really about food or vomiting, but rather about germs. Kylie's mother notes that Kylie has some other behaviors similar to this - she seems to need to count the sidewalk cracks and she seems to need to have all of her clothes perfectly ironed without any wrinkles at all or she has a complete meltdown.  But these other behaviors are not as severe or all consuming as the problem related to germs and hand washing... What is the MOST LIKELY disorder that Kylie has?

Step-by-Step Analysis
The stem describes a 10-year-old with intrusive fears about germs (obsessions) and repetitive behaviors aimed at reducing distress (compulsions): washing hands for long periods with soap and hot water, using a mechanical grabber to avoid touching things, and prevented eating with utensils to avoid contact. The compulsions are time-consuming, cause physical harm (raw, red hands, sores), and the person recognizes the behaviors as excessive, yet feels driven to perform them to relieve anxiety. In addition, there are other related ritualistic concerns (counting sidewalk cracks, insisting on perfectly ironed clothes) that share a common pattern of obsessions and compulsions or compulsive rituals. This constellation is classic for obsessive-compulsive disorder (OCD).
